• Once you click the link it will take you to our admission portal where you would then click "APPLY HERE"
• Choose "I am a Consultant"
• Choose what facility you are referring too, it's okay if it's both just choose one for the sign-up process.
• You would create your own Referral login so that way you can sign on every time you want to send us a referral, you can add the client, upload their eval.
• You will be able to see the status of the client's application.
• PLEASE PROVIDED EVAL

• Upon clicking the link, you are then on the portal
• Click the "APPLY HERE"
• Choose whether you are applying for yourself or if you are a family member applying for someone.
• Enter all your Demographics
• Please have your Chemical Dependency Evaluation available to upload into the chart, (If you do not have an eval and need assistance getting one done please use the contact us form and we will help get you set up for a CDE)
• Once submitted you can log back on to see communication and you can reach out to us directly from the portal, this provides individuals with faster communication.
